Please use this center to help prepare for your upcoming visit.

Please arrive 5-10 minutes prior to your appointment time. Below are the forms we need to ensure you receive the best comprehensive care we can provide.  Simply print out the forms below and arrive to your appointment with them already filled out to help you streamline the check-in process.



If you are new to our office and have not been dilated previously, you may wish to bring along a driver until you know how it will affect your vision. Dilation is an important part of eye health exams and doesn’t typically affect distance vision, but it can cause light sensitivity and blurred near vision.

Please bring with you:

  • All your insurance cards and information.
  • Previous prescription information, current glasses you are using, including over the counter readers
  • Current contact lens prescription information or boxes
  • Current medication list

We are preferred providers for the following vision and medical insurances. This is not an all inclusive list, please call our office if you do not see your plan to check if we are providers.
